Since the beginning of the pandemic and health measures adopted in the country, the Government of Peru has issued several standards to boost the use of health-focused Information and Communication Technologies (ICTs).<\/h2>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t

This article details the challenges of the COVID-19 pandemic, which has yet been an opportunity for the development of telemedicine and Digital Health. In Peru since 2005 the National Telehealth Plan was established and later the Telehealth Technical Standard was approved by the Ministry of Health (MINSA) in 2008. However, it was until April 2016 that the Telehealth Framework Law was published after three years of writing and implementation work.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t

Later that year, it was amended by the Legislative Decree in December, extending the concept of telehealth to the one it defines as follows: remote health service provided by competent health personnel, through Information and Communication Technologies \u2013 ICT, to make these services and their related services accessible mainly to users in rural areas or with limited resolution capacity. This service is carried out taking into account the following development axes of Telehealth: the provision of health services; the management of health services; information, education and communication to the population about health services; and capacity building for health personnel, among others.\"<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t

In 2017 the Directorate General of Telehealth, Reference and Emergency, was created, the body responsible for regulating and implementing policies on telehealth. These advances in favor of Digital Health practices were important to the challenges that would arise this year. In 2020, following the emergency situation arising from the COVID-19 pandemic, Supreme Decree No. 013-2020-SA was published, which established that MINSA regulate procedures for \"performing telemedicine services with special emphasis on remote medical guidance, tele-monitoring and mental health during the Health Emergency\". Similarly, on 10 May, the definitions of telehealth and telemedicine were changed again to strengthen the reaches of Digital Health: \"the remote health service provided by competent health personnel, through ICT, so that they are accessible and timely (sic) to the population\".<\/p>

MINSA began providing medical tele guidance in different specialties and in its first two weeks recorded more than 4,000 requests for care. A telephone line and a mobile application were enabled, this second tool was also one of the axes of the strategy against COVID-19 in the country. Through the application, health authorities monitored patients at risk of COVID-19 and infected patients.<\/p>

For its part EsSalud (Social Security Institution of Peru), approved the General Management Directive No. 12: Telemedicine in Patients with Chronic disease<\/strong>, and in this way implemented remote consultations with main care in patients with chronic diseases. EsSalud's actions were coordinated through the National Telemedicine Center (CENATE).<\/p>

Other institutions such as the Medical College of Peru (CMP) developed coronavirus self-assessment tools to help determine the people who should be tested according to the symptoms experienced. Similarly, CMP created a digital platform called Health to a Click to record patients' medical records, and make it easier for doctors to work.<\/p>

However, despite the great efforts of different public and even private bodies, in Peru there is a gap related to Internet connectivity, since only 2 thousand 108 health service providers are part of the National Telemedicine Network, equivalent to 25.5%.<\/p>

The article mentions that \"the mass adoption of telehealth also requires a significant change in the management effort and redesign of existing care models\", which also poses a major challenge. In terms of challenges, universities play an important role in the development of health professionals and in the creation of initiatives in Digital Health.<\/p>

Digital Health in Peru, even during the pandemic, is in full growth and has a great margin of growth thanks to advances in the legal framework and in the creation of regulatory bodies of the various telehealth programs offered by public health services.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t

Desde el comienzo de la pandemia y las medidas sanitarias adoptadas en el pa\u00eds, el Gobierno de Per\u00fa ha emitido varias normas para impulsar el uso de Tecnolog\u00edas de Informaci\u00f3n y Comunicaci\u00f3n (TIC) enfocadas en salud. Este art\u00edculo detalla los desaf\u00edos que significa la pandemia de COVID-19, y que a pesar de ello ha significado una oportunidad para el desarrollo de la telemedicina y Salud Digital. En Per\u00fa desde 2005 se estableci\u00f3 el Plan Nacional de Telesalud y posteriormente la Norma T\u00e9cnica de Telesalud fue aprobada por el Ministerio de Salud (MINSA) en 2008. Sin embargo, fue hasta abril del 2016 cuando se public\u00f3 la Ley Marco de Telesalud luego de tres a\u00f1os de trabajo de redacci\u00f3n e implementaci\u00f3n. M\u00e1s tarde ese a\u00f1o, fue modificada por el Decreto Legislativo en diciembre, ampliando el concepto de telesalud a la que define de la siguiente forma: servicio de salud a distancia prestado por personal de salud competente, a trav\u00e9s de las Tecnolog\u00edas de la Informaci\u00f3n y de la Comunicaci\u00f3n \u2013 TIC, para lograr que estos servicios y sus relacionados, sean accesibles principalmente a los usuarios en \u00e1reas rurales o con limitada capacidad resolutiva. Este servicio se efect\u00faa considerando los siguientes ejes de desarrollo de la Telesalud: la prestaci\u00f3n de los servicios de salud; la gesti\u00f3n de los servicios de salud; la informaci\u00f3n, educaci\u00f3n y comunicaci\u00f3n a la poblaci\u00f3n sobre los servicios de salud; y el fortalecimiento de capacidades al personal de salud, entre otros\u201d. En 2017 fue creada la Direcci\u00f3n General de Telesalud, Referencia y Urgencias, el organismo responsable de regular e implementar pol\u00edticas sobre telesalud. Estos avances a favor de las practicas de Salud Digital, fueron importantes para los retos que se presentar\u00edan este a\u00f1o. En 2020 a ra\u00edz la situaci\u00f3n de emergencia derivada de la pandemia por COVID-19 se public\u00f3 el Decreto Supremo N.\u00ba 013-2020-SA que estableci\u00f3 que el MINSA regulara los procedimientos para “realizar los servicios de telemedicina con especial \u00e9nfasis en la tele orientaci\u00f3n m\u00e9dica a distancia, tele monitoreo y salud mental durante la Emergencia Sanitaria\u201d. De la misma manera, el 10 de mayo se modificaron nuevamente las definiciones de telesalud y telemedicina para fortalecer los alcances de la Salud Digital: \u201cel servicio de salud a distancia prestado por personal de la salud competente, mediante las TIC, a fin de que sean accesibles y oportunos (sic) a la poblaci\u00f3n\u201d. El MINSA comenz\u00f3 a realizar tele orientaciones m\u00e9dicas en distintas especialidades y en sus primeras dos semanas registraron m\u00e1s de 4 mil solicitudes de atenci\u00f3n. Se habilit\u00f3 una l\u00ednea telef\u00f3nica y un aplicativo m\u00f3vil, esta segunda herramienta fue adem\u00e1s uno de los ejes de la estrategia contra el COVID-19 en el pa\u00eds. A trav\u00e9s de la aplicaci\u00f3n las autoridades de salud realizaron monitoreos a los pacientes con riesgo de COVID-19 y a pacientes contagiados. Por su parte EsSalud (Instituci\u00f3n de seguridad Social de Per\u00fa), aprob\u00f3 la Directiva de Gerencia General No 12: Telemedicina en Pacientes con Enfermedad Cr\u00f3nica o Continuador, y de esta forma implementaron las consultas a distancia con principal atenci\u00f3n en pacientes con enfermedades cr\u00f3nicas. Las acciones de EsSalud, fueron coordinadas a trav\u00e9s del Centro Nacional de Telemedicina (CENATE). Otras instituciones como el Colegio M\u00e9dico del Per\u00fa (CMP), desarrollaron herramientas de autoevaluaci\u00f3n de coronavirus, para ayudar a determinar las personas que deben hacerse pruebas de acuerdo a los s\u00edntomas experimentados. De igual forma el CMP cre\u00f3 una plataforma digital llamada Salud a un Clic para registrar la historia cl\u00ednica de los pacientes, y facilitar el trabajo a los m\u00e9dicos. Sin embargo, a pesar de los grandes esfuerzos de distintos organismos p\u00fablicos e incluso privados, en Per\u00fa existe una brecha relacionada a la conectividad a Internet, pues solo 2 mil 108 instituciones prestadoras de servicios de salud forman parte de la Red Nacional de Telemedicina, lo que equivale a un 25,5%. El art\u00edculo menciona que “la adopci\u00f3n masiva de la telesalud tambi\u00e9n requiere un cambio significativo en el esfuerzo de gesti\u00f3n y el redise\u00f1o de los modelos de atenci\u00f3n existentes”, lo que tambi\u00e9n representa un desaf\u00edo importante. En cuanto a los desaf\u00edos, las universidades juegan un papel importante en el desarrollo de profesionales de la salud y en la creaci\u00f3n de iniciativas en Salud Digital. La Salud Digital en Per\u00fa aun durante la pandemia, se encuentra en pleno crecimiento y cuenta con un gran margen de crecimiento gracias a los avances en marco legal y en la creaci\u00f3n de organismos reguladores de los diversos programas en telesalud que ofrecen los servicios p\u00fablicos de salud.
