Researchers of the <em>Military and Emergency Medicine<\/em> (MEM), from <em>Uniformed Services University<\/em> of Maryland, USA, participated in the analysis, which states that their military forces have expanded available telemedicine systems by using low-cost mobile technologies for the response to any type of disaster or emergency. These technologies have allowed intensivists despite the high volume of patients during the pandemic, to have the tools to consult and provide medical assistance at a distance, especially to those patients at increased risk of a serious respiratory disease due to COVID-19.<\/p><p>However, the biggest challenge for health systems is the future of Digital Health in the face of new health emergencies, which is why the NETCCN proposal emerges, which would function as a stand-alone medical information system for the management, creation and coordination of critical care virtual rooms. The combination of live audiovisual channels would allow expansion to remote areas, a permanent system of this magnitude could prevent the transfer of patients to distant hospitals. Advanced analytics tools, AI, and visualization and data would provide tools for rapid action in patient treatment.<\/p><p>The NETCCN proposal is defined in four phases: in the first two phases the system would operate within a specific geographical area, with one or two virtual rooms, for the exclusive use of local physicians. For Phase III, the system would operate across multiple states and for phase IV, it could operate in all 50 states and territories, for the preliminary and temporary creation of these virtual rooms would include the use of local gyms, convention centers, among others.<\/p><p>Among the challenges described in the publication, there are undoubtedly the legal obstacles that must be solved to establish a massive Telehealth network in a country as extensive as the US. Investigadores de distintas universidades y centros investigaci\u00f3n de EEUU, presentaron un an\u00e1lisis sobre el panorama de la Salud Digital ante futuras emergencias titulado &#8220;Advanced Digital Health Technologies for COVID-19 and Future Emergencies&#8221; en el que se analizan las herramientas actuales de salud digital como telemedicina y telecuidado cr\u00edtico. El art\u00edculo describe c\u00f3mo las tecnolog\u00edas en Salud Digital son utilizadas o pueden ser utilizadas para la mitigaci\u00f3n del virus, proponiendo el National Emergency Tele-Critical Care Network (NETCC), un organismo de gobierno que combinar\u00eda la Salud Digital para enfrentar las futuras crisis sanitarias o clim\u00e1ticas. Con la pandemia de COVID-19 y la detecci\u00f3n de los puntos d\u00e9biles y fuertes del sistema de salud norteamericano, se abre una nueva oportunidad para la integraci\u00f3n total de la Salud Digital en los servicios de salud. Investigadores del Military and Emergency Medicine (MEM), de la Uniformed Services University de Maryland, EEUU, participaron en el an\u00e1lisis, en el que exponen que sus fuerzas militares, han expandido los sistemas de telemedicina disponibles, al utilizar tecnolog\u00edas m\u00f3viles de bajo costo para la respuesta a cualquier tipo de desastres o emergencias. &nbsp;Adem\u00e1s, en el art\u00edculo enfatizan en que la Salud Digital y sus herramientas no se limitan al uso de telemedicina, Telesalud, o historiales cl\u00ednicos electr\u00f3nicos, sino que tambi\u00e9n incluye el Big Data, la Inteligencia Artificial, sensores inal\u00e1mbricos, entre otras herramientas. La primera parte del texto se refiere a la descripci\u00f3n de las herramientas actuales de Salud Digital, las que hab\u00edan sido utilizadas previamente, y las que a ra\u00edz de la pandemia pasaron por un proceso fugaz para su aplicaci\u00f3n y uso. Telemedicina, Telecuidado cr\u00edtico, Tele asesor\u00eda Nivelada, Informaci\u00f3n para la toma de decisiones o Dispositivos que permiten el cuidado con poco contacto y aislamiento, son algunas de las tecnolog\u00edas y herramientas actuales. Estas han permitido que intensivistas a pesar del alto volumen de pacientes durante la pandemia, tengan las herramientas para consultar y brindar asistencia m\u00e9dica a distancia, sobre todo, a aquellos pacientes con mayor riesgo de padecer una enfermedad respiratoria grave derivada del COVID-19. No obstante, el reto mayor para los sistemas de salud, es el futuro de la Salud Digital ante nuevas emergencias sanitarias, es por eso que surge la propuesta de la NETCCN, que funcionar\u00eda como un sistema de informaci\u00f3n m\u00e9dica \u00fanico para la administraci\u00f3n, creaci\u00f3n y coordinaci\u00f3n de salas virtuales de cuidado cr\u00edtico. La combinaci\u00f3n de canales audiovisuales en directo permitir\u00eda la expansi\u00f3n a zonas remotas, un sistema permanente de esa magnitud podr\u00eda prevenir las trasferencias de pacientes a hospitales lejanos. Herramientas de anal\u00edtica avanzada, IA, y visualizaci\u00f3n y datos brindar\u00edan herramientas para la acci\u00f3n r\u00e1pida en el tratamiento de pacientes. La propuesta de la NETCCN se define en cuatro fases: en las primeras dos fases el sistema operar\u00eda dentro de un \u00e1rea geogr\u00e1fica espec\u00edfica, con una o dos salas virtuales, para uso exclusivo de m\u00e9dicos locales. Para la fase III, el sistema operar\u00eda a trav\u00e9s de m\u00faltiples estados y para la fase IV, podr\u00eda operar en los 50 estados y territorios, para la creaci\u00f3n preliminar y temporal de estas salas virtuales se contemplar\u00eda el uso de gimnasios locales, centros de convenciones, entre otros. Entre los retos que describen en la publicaci\u00f3n, existen sin duda los obst\u00e1culos legales que deben resolverse para establecer una red masiva de Telesalud en un pa\u00eds tan extenso como EEUU. Adem\u00e1s del software y hardware necesario para la implementaci\u00f3n de sistemas digitales en hospitales, y la adquisici\u00f3n de unidades m\u00f3viles de salud, que seg\u00fan describen en el art\u00edculo, estas soluciones son f\u00e1ciles de aplicar y de mantener, ya que su costo no es tan elevando, y sobre todo son ideales para situaciones de cat\u00e1strofes naturales como huracanes o tormentas.
