The implementation process of the Electronic Medical Record of Uruguay is one of the main examples of digitization of the health system in Latin America.<\/h2>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t

The path towards the implementation of the National Electronic Clinical Record (HCEN) in Uruguay began in 2007, when the government began a health reform process that led to the creation of the National Integrated Health System (SNIS).<\/p>

\u201cThe SNIS is thinking about the consecration of the right to health by all the country's inhabitants. From this conception, it is a right to know the trajectory of the health-disease process throughout the life of any of the people who are part of the system, and this right is enshrined in the SNIS laws as an essential human right. The EHR is one of the most suitable tools to protect it\u201d, explained Mar\u00eda Julia Mu\u00f1oz, Minister of Public Health of Uruguay between 2005 and 2010.<\/p>

In this way, the SNIS seeks universal health coverage, through public and private providers. This implies that all health institutions provide comprehensive care, that is, the same benefits, this would facilitate the implementation of the HCEN.<\/p>

In this sense, towards the year 2012, a new process began that led to the creation of Salud.uy, a digital health initiative of the Uruguayan government, which would give rise to the design of strategies to develop and implement an HCEN. In addition to determining the medical informatics guidelines and technical and regulatory terms.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t

Created in 2005 at the beginning of the entire project, the Electronic Government and Knowledge Information Society Agency (Agesic), and Salud.uy were key to the deployment of the strategic lines for the design and implementation of the HCEN in the country. .<\/p>

In 2014, after a fiber optic deployment strategy that began in 2011, they managed to reach half a million active fiber optic connections. And in 2015, the National Directorate of Civil Identification began to issue Digital Identity Cards and electronic signatures.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t

On the other hand, the IDB contributed approximately 21 million USD for the implementation of this project, through three loans of 6 million in 2013, 2017 and 2021. The loans had different objectives, first the establishment of a digital network of service providers of service, second to consolidate the digital platform for the exchange of clinical information and the third will seek to deepen the first two objectives and increase the use of data from medical records for decision making.<\/p>

In 2017, Ordinance No. 1085 of the Ministry of Public Health regulated the decree that established the HCEN adoption plan, which all health institutions, public and private, had to comply with. This represented a milestone in the digital transformation of the health sector in Uruguay.<\/p>

And in the Accountability Law (19,670), "it established that health providers should join the HCEN and defined that people join by registering their information through the provider," according to the Inter-American Development Bank (IDB). . And in 2019, decree 122\/019 regulated data management by users and health service providers on the HCEN platform.<\/p>

Finally, after various ordinances by the MSP during 2020, the following year Law No. 19996 established that the information generated in the HCEN would be the only valid source to certify the health situation of all workers, and it was agreed to December 31, 2022, as the deadline for service providers to comply with said order.<\/p>

Likewise, the IDB published the document Implementation of the National Electronic Medical Record of Uruguay,<\/em> which makes an extensive journey through the HCEN process in Uruguay, and recognizes it as one of the most important initiatives in Latin America in terms of Digital Health.<\/p>

El proceso de implementaci\u00f3n de la Historia Cl\u00ednica Electr\u00f3nica de Uruguay, es uno de los principales ejemplos de digitalizaci\u00f3n del sistema de salud en Latinoam\u00e9rica. El camino hacia la implementaci\u00f3n de la Historia Cl\u00ednica Electr\u00f3nica Nacional (HCEN) en Uruguay comenz\u00f3 desde 2007, cuando el gobierno inici\u00f3 un proceso de reforma sanitaria que deriv\u00f3 en la creaci\u00f3n del Sistema Nacional Integrado de Salud (SNIS). \u201cEl SNIS est\u00e1 pensando en la consagraci\u00f3n del derecho a la salud por parte de todos los habitantes del pa\u00eds. Desde esa concepci\u00f3n, es un derecho conocer la trayectoria del proceso salud-enfermedad a lo largo de la vida de cualquiera de las personas que son parte del sistema, y ese derecho est\u00e1 consagrado en las propias leyes del SNIS como derecho humano esencial. La HCE es una de las herramientas m\u00e1s aptas para protegerlo\u201d, explic\u00f3 en su momento Mar\u00eda Julia Mu\u00f1oz, ministra de Salud P\u00fablica de Uruguay Entre 2005 y 2010. De esta forma el SNIS busca la cobertura universal en salud, a trav\u00e9s de proveedores p\u00fablico y privados. Esto implica que todas las instituciones de salud brinden atenci\u00f3n integral, es decir las mismas prestaciones, esto lograr\u00eda facilitar la implementaci\u00f3n de la HCEN. En este sentido, hacia el a\u00f1o 2012, comenz\u00f3 un nuevo proceso que condujo a la creaci\u00f3n de Salud.uy, iniciativa de salud digital del gobierno Uruguay, que dar\u00eda origen al dise\u00f1o de estrategias para desarrollar e implementar una HCEN. Adem\u00e1s de determinar los lineamientos de inform\u00e1tica m\u00e9dica y t\u00e9rminos t\u00e9cnicos y regulatorios. Creada en 2005 al inicio de todo el proyecto, la Agencia de Gobierno Electr\u00f3nico y Sociedad de la Informaci\u00f3n del Conocimiento (Agesic), y Salud.uy fueron claves para el despliegue de las l\u00edneas estrat\u00e9gicas para el dise\u00f1o e implementaci\u00f3n de la HCEN en el pa\u00eds. En 2014, luego de una estrategia de despliegue de fibra \u00f3ptica que comenz\u00f3 en 2011, lograron alcanzar medio mill\u00f3n de conexiones activas de fibra \u00f3ptica. Y en 2015, la direcci\u00f3n Nacional de Identificaci\u00f3n Civil, comenz\u00f3 a emitir Cedulas de Identidad Digital y firmas electr\u00f3nicas. Por otra parte, el BID, aport\u00f3 aproximadamente 21 millones USD, para la implementaci\u00f3n de este proyecto, a trav\u00e9s de tres prestamos de 6 millones en 2013, 2017 y 2021. Los prestamos tuvieron diferentes objetivos, primero el establecimiento de una red digital de prestadores de servicio, segundo consolidar la plataforma digital de intercambio de informaci\u00f3n cl\u00ednica y el tercero buscar\u00e1 profundizar los primeros dos objetivos y aumentar el uso de datos de registros m\u00e9dicos para la toma de decisiones. En 2017, la ordenanza No. 1085 del Ministerio de Salud P\u00fablica, reglament\u00f3 el decreto que establec\u00eda el plan de adopci\u00f3n de la HCEN, que deb\u00edan cumplir todas las instituciones de salud, p\u00fablicas y privadas. Esto represent\u00f3 un hito en la transformaci\u00f3n digital del sector sanitario en Uruguay. Y en Ley de Rendici\u00f3n de Cuentas (19.670), \u201cestableci\u00f3 que las entidades prestadoras de salud deb\u00edan incorporarse a la HCEN y defini\u00f3 que las personas se integren registrando su informaci\u00f3n a trav\u00e9s del prestador”, seg\u00fan explica el Banco Interamericano de Desarrollo (BID). Y en 2019, el decreto 122\/019, reglament\u00f3 la gesti\u00f3n de datos por parte de los usuarios y los prestadores de servicios de salud en la plataforma de la HCEN. Finalmente, tras diversas ordenanzas por parte del MSP durante 2020, al a\u00f1o si siguiente la Ley No. 19996, estableci\u00f3 que la informaci\u00f3n generada en las HCEN ser\u00edan la \u00fanica fuente v\u00e1lida para acreditar la situaci\u00f3n de salud de todos los trabajadores, y se acord\u00f3 el 31 de diciembre de 2022, como fecha l\u00edmite para que los prestadores de servicios acataran dicha orden. Asimismo, el BID public\u00f3 el documento Implementaci\u00f3n de la Historia Cl\u00ednica Electr\u00f3nica Nacional de Uruguay, que realiza un recorrido extenso a trav\u00e9s del proceso de la HCEN en Uruguay, y la reconoce como una de las iniciativas m\u00e1s importantes de Latinoam\u00e9rica en cuanto a Salud Digital.
