The Government of Uruguay has gradually developed its own digital health ecosystem in recent years through the Salud.uy strategy.<\/h2>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t

At the end of 2012, the Uruguayan government presented the Salud.uy strategy to promote the strengthening and use of information technologies in the health sector. It is a joint initiative between the Presidency of the Republic, the Ministry of Public Health (MSP), the Ministry of Economy and Finance (MEF) and the Electronic Government and Information and Knowledge Society Agency (Agesic).<\/p>

As a result of the Salud.uy program, the government has defined standards and guidelines for medical informatics and regulations on digital health. This has allowed the creation of the National Electronic Medical Record (HCEN), which is part of the National Integrated Health System (SNIS).<\/p>

After almost ten years of its implementation, Salud.uy has a team of medical informatics specialists and multidisciplinary work groups, which are in charge of generating these regulations and guidelines.<\/p>

The HCEN is undoubtedly Salud.uy's flagship project, which has been the gateway for the implementation of new projects and standards in the health sector. For example, through the MiHCD site, the population can access all their clinical information, regardless of whether it includes data from various facilities.<\/p>

Pablo Orefice, former director of the Salud.uy initiative, and Luis Tejerina, equal leader in the Social Protection and Health Division of the Inter-American Development Bank (IDB), detailed the importance of the HCEN in the fight against COVID-19 , since the infrastructure and the digital ecosystem were ready to share information between health establishments and medical professionals.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t

\u201cIt was possible to share information, for example, from tests<\/em> of laboratories or beds available through the same infrastructure used for the HCEN\u201d, explain the specialists.<\/p>

Likewise, Tejerina and Orefice published a case study of the HCEN through Social Digital, an IDB initiative, for the transformation of social services. In this sense, they shared ten tips on digital transformation in health, taking the Salud.uy project as a reference:<\/p>

  1. Always place the patient at the center of the design, emphasizing equity.<\/li>
  2. Build a vision of what you want to achieve with everyone and for all the actors in the system (public and private sector, academia, professionals, citizens, etc.).<\/li>
  3. Have a team of professionals specialized in digital health with clear objectives.<\/li>
  4. Develop a strategy that converts the national vision into an implementation plan as a state policy, aligned with the country's health system.<\/li>
  5. Approve the necessary regulations so that the technological tools can be used and the applicable model.<\/li>
  6. Accompany the ecosystem with knowledge, workshops, artifacts as public goods, collective construction of documentation to carry out their own digital transformation processes in organizations.<\/li>
  7. Think and create an incentive scheme that pushes suppliers to make the necessary investments (economic incentives, legal obligations, penalties, etc.).<\/li>
  8. Generate early victories, as part of the strategy of long transformation processes and deploy a national architecture that supports future processes.<\/li>
  9. Invest in continuous monitoring of progress, for example through recurring surveys.<\/li>
  10. Remember the human factor, both people who are involved in the digital transformation of the sector, and citizens in general must be accompanied through change management plans.<\/li><\/ol>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t

    El Gobierno de Uruguay ha desarrollado poco a poco su propio ecosistema digital en salud durante los \u00faltimos a\u00f1os a trav\u00e9s de la estrategia Salud.uy. A finales del 2012, el gobierno de Uruguay present\u00f3 la estrategia Salud.uy, para promover el fortalecimiento y uso de tecnolog\u00edas de la informaci\u00f3n en el sector salud. Se trata de una iniciativa conjunta entre la Presidencia de la Rep\u00fablica, el Ministerio de Salud P\u00fablica (MSP), el Ministerio de Econom\u00eda y Finanzas (MEF) y la Agencia de Gobierno Electr\u00f3nico y Sociedad de la Informaci\u00f3n y del Conocimiento (Agesic). A ra\u00edz del programa de Salud.uy el gobierno ha definido est\u00e1ndares y lineamientos de inform\u00e1tica m\u00e9dica y regulaciones sobre salud digital. Esto ha permitido la creaci\u00f3n de la Historia Cl\u00ednica Electr\u00f3nica Nacional (HCEN), que forma parte del Sistema Nacional Integrado de Salud (SNIS). Tras casi diez a\u00f1os de su implementaci\u00f3n Salud.uy, cuenta con un equipo de especialistas de inform\u00e1tica m\u00e9dica y grupos de trabajo multidisciplinarios, que se encargan de la generaci\u00f3n de estos reglamentos y lineamientos. El HCEN es sin duda el proyecto insignia de Salud.uy, lo que ha sido la entrada para la implementaci\u00f3n de nuevos proyectos y est\u00e1ndares en el sector salud. Por ejemplo, a trav\u00e9s del sitio MiHCD, la poblaci\u00f3n puede acceder a toda su informaci\u00f3n cl\u00ednica, sin importar si incluye datos de diversos establecimientos. Pablo Orefice, ex director de la iniciativa Salud.uy, y Luis Tejerina, ecualiza l\u00edder en la Divisi\u00f3n de Protecci\u00f3n Social y Salud del Banco Interamericano de Desarrollo (BID), detallaron la importancia que tuvo el HCEN en la lucha contra el COVID-19, ya que la infraestructura y el ecosistema digital se encontraban listos para compartir informaci\u00f3n entre establecimientos de salud y profesionales m\u00e9dicos. \u201cFue posible compartir informaci\u00f3n, por ejemplo, de tests de laboratorio o camas disponibles a trav\u00e9s de la misma infraestructura utilizada para la HCEN\u201d, explican los especialistas. Asimismo, Tejerina y Orefice publicaron un estudio de caso de la HCEN a trav\u00e9s de Social Digital, iniciativa del BID, para la transformaci\u00f3n de los servicios sociales. Construir una visi\u00f3n de lo que se quiere lograr con todos y para todos los actores del sistema (sector p\u00fablico y privado, academia, profesionales, ciudadanos, etc.). Tener un equipo de profesionales especializados en salud digital con objetivos claros. Desarrollar una estrategia que convierta la visi\u00f3n nacional en un plan de implementaci\u00f3n como pol\u00edtica de estado, alineada al sistema de salud del pa\u00eds. Aprobar la normativa necesaria para que las herramientas tecnol\u00f3gicas puedan ser utilizadas y el modelo aplicable. Acompa\u00f1ar al ecosistema con conocimiento, talleres, artefactos como bienes p\u00fablicos, construcci\u00f3n colectiva de documentaci\u00f3n para llevar adelante sus propios procesos de transformaci\u00f3n digital en las organizacionales. Pensar y crear un esquema de incentivos que empuje a los proveedores a realizar las inversiones necesarias (incentivos econ\u00f3micos, obligaciones legales, penalidades, etc.). Generar victorias tempranas, como parte de la estrategia de los procesos largos de transformaci\u00f3n y desplegar una arquitectura nacional que de soporte a futuros procesos. Invertir en monitoreo continuo del progreso, por ejemplo, a trav\u00e9s de encuestas recurrentes. Recordar el factor humano, tanto las personas que est\u00e9n involucradas en la transformaci\u00f3n digital del sector, como los ciudadanos en general deben ser acompa\u00f1adas a trav\u00e9s de planes de gesti\u00f3n del cambio.
