The third day of activities of the RECAINSA 2021 Digital Health Forum, had the participation of Dr. V\u00edctor Medina, president of HL7 Mexico Chapter, who spoke about the situation in the adoption of said standard in Mexico.<\/h2>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t

HL7 or Health Level Seven International is an organization that develops international standards and interoperability solutions for health data. It is one of the most widely used standards worldwide in the health sector for data management.<\/p>

In the first decade of the 2000s, the HL7 Mexico chapter was created as part of a government initiative. However, in 2012 it was discontinued by the authorities. This situation removed this standard from the panorama of Digital Health in Mexico in subsequent years. Thus, there was a lack of updating guidelines for the application of the standard in the country.<\/p>

In September 2020, the Mexico Standard Association was created, to resume the HL7\/FHIR Chapter in Mexico. Although it is a private sector initiative, they have the support of various academic organizations, and the participation of public officials. And most importantly, they have the support of HL7 International.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t

The association has work tables on: digital prescription, medical devices, relationship with the government, link with medical associations, telecare, link with insurer, unification of EAN codes, link and funding, electronic clinical file and interoperability standards.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t

Dr. Medina, who participates in the interoperability standards table, explained that, in Mexico, there have been two Official Mexican Standards (NOM) referring to the standards for the exchange of information in health and interoperability, the NOM-024-2010 that later it was replaced by NOM-024-2012. Likewise, after the impact of the pandemic, there are currently only three certifications in Mexico for the Health Information System Certification.<\/p>

Exchange of health information is not the same as interoperability<\/strong><\/p>

"When we talk about interoperability, we can use the information that they exchanged with us in a transparent way, it is not necessary to clean it, or modify it to be able to use it." Otherwise, with the exchange of health information, it is possible to change the formats in order to interpret it or read it in order to use it, explains Dr. Medina.<\/p>

HL7\/FHIR establishes communication standards in specific formats through APIS (compatible with XL or JSON). In addition, it is a free and direct protocol.<\/p>

HL7 implementations in Mexico <\/strong><\/p>

In 2006, it was implemented in the Mexican Institute of Social Security (IMSS), to integrate hospital information services and the electronic clinical file. Several implementations have recently been carried out within the same institute: in 2015 for extramural hemodialysis information systems; in 2017 for clinical laboratories and blood banks; and in 2020 for the network of epidemiological surveillance and research laboratories.<\/p>

El tercer d\u00eda de actividades del Foro de Salud Digital RECAINSA 2021, cont\u00f3 con la participaci\u00f3n del Dr. V\u00edctor Medina, presidente de HL7 Cap\u00edtulo M\u00e9xico, quien expuso sobre la situaci\u00f3n en la adopci\u00f3n de dicho est\u00e1ndar en M\u00e9xico. HL7 o Health Level Seven International es una organizaci\u00f3n que desarrolla est\u00e1ndares y soluciones de interoperabilidad internacionales para datos de salud. Es uno de los est\u00e1ndares m\u00e1s utilizados a nivel mundial en el sector salud para el manejo de datos. En la primera d\u00e9cada de los a\u00f1os 2000, se cre\u00f3 el cap\u00edtulo de HL7 M\u00e9xico como parte de una iniciativa gubernamental. Sin embargo, en 2012 fue descontinuado por las autoridades. Esta situaci\u00f3n alej\u00f3 este est\u00e1ndar del panorama de la Salud Digital en M\u00e9xico los a\u00f1os posteriores. De esta forma hubo falta de actualizaci\u00f3n en lineamientos para la aplicaci\u00f3n del est\u00e1ndar en el pa\u00eds. En septiembre del 2020, se cre\u00f3 la Asociaci\u00f3n est\u00e1ndar M\u00e9xico, para retomar el Cap\u00edtulo de HL7\/FHIR en M\u00e9xico. A pesar de que se trata de una iniciativa del sector privado, cuentan con el respaldo de diversas organizaciones acad\u00e9micas, y participaci\u00f3n de funcionarios p\u00fablicos. Y lo m\u00e1s importante, cuentan con el respaldo de HL7 Internacional. La asociaci\u00f3n cuenta con mesas de trabajo en: receta digital, dispositivos m\u00e9dicos, relaci\u00f3n con gobierno, enlace con asociaciones m\u00e9dicas, teleasistencia, vinculaci\u00f3n con aseguradora, unificaci\u00f3n de c\u00f3digos EAN, vinculaci\u00f3n y fondeo, expediente cl\u00ednico electr\u00f3nico y est\u00e1ndares de interoperabilidad. El Dr. Medina, que participa en la mesa de est\u00e1ndares de interoperabilidad, explic\u00f3 que, en M\u00e9xico han existido dos Normas Oficiales Mexicanas (NOM) referentes a los est\u00e1ndares de intercambio de informaci\u00f3n en salud e interoperabilidad, la NOM-024-2010 que posteriormente fue remplazada por la NOM-024-2012. Asimismo, tras el impacto de la pandemia, actualmente solo existen tres certificaciones en M\u00e9xico para la Certificaci\u00f3n de Sistema de Informaci\u00f3n en Salud. Intercambio de informaci\u00f3n en salud no es lo mismo que interoperabilidad \u201cCuando hablamos de interoperabilidad, podemos usar la informaci\u00f3n que nos intercambiaron de manera transparente, no es necesario limpiarla, ni modificarla para poder usarla\u201d. Caso contrario con el intercambio de informaci\u00f3n en salud es posible cambiar los formatos para poder interpretarla o leerla para poder utilizarla, explica el Dr. Medina. HL7\/FHIR establece est\u00e1ndar de comunicaci\u00f3n en formatos espec\u00edficos a trav\u00e9s de APIS (compatible con XL o JSON). Adem\u00e1s, es un protocolo gratuito y directo. Implementaciones de HL7 en M\u00e9xico En 2006 logr\u00f3 implementarse en el Instituto Mexicano del Seguro Social (IMSS), para integrar servicios de informaci\u00f3n hospitalaria y el expediente cl\u00ednico electr\u00f3nico. Recientemente se han realizado varias implementaciones dentro del mismo instituto: en 2015 para sistemas de informaci\u00f3n de hemodi\u00e1lisis extramuros; en 2017 para laboratorios cl\u00ednicos y banco de sangre; y en 2020 para la red de laboratorios de vigilancia e investigaci\u00f3n epidemiol\u00f3gica.
