This February, the Inter-American Development Bank (IDB) published the public proposal VISION 2025 Reinvesting in the Americas: A Decade of Opportunities, <\/em>which seeks, through a digital economy, to facilitate access to the adoption of digital technologies and foster innovation. The IDB also published a document on electronic health record (EHR) systems in April last year. In the publication Electronic Health Record Systems: Definitions, evidence and practical recommendations for Latin America and the Caribbean<\/em>, they address issues such as eHealth, digital transformation and experiences on EHR implementation in health systems.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t

On the other hand, in a blog post, the IDB presents as an example the implementation of an EHR system in the state of Bahia, where the Bahia State Health Secretariat (SESAB) uses a free software system called AGHUse. Through this experience, Bertha Brice\u00f1o, Fernanda Camera, Jennifer Nelson and Luis Tejerina, IDB specialists in Digital Health and digital transformation listed ten recommendations or lessons to be taken into account about EHR systems:<\/p>

  1. There is no such thing as free software: The authors explain that open software does not imply zero expenditure, as it requires a team of professionals to accompany its implementation and maintenance.<\/li>
  2. Governance is key: \"In this format of community software development, it is important to have a governance system that clearly defines the roles and commitments of each partner, the procedures for requesting adjustments and solving problems,\" explain the authors.<\/li>
  3. It is important to create a sustainable and scalable model: A sustainability plan must be included, establishing percentages of expected developments.<\/li>
  4. The software requires adaptations, especially for specialized areas: The software used should be selected taking into account the particular processes and needs that each department will use, for example maternity, oncology, geriatric services, etc.<\/li>
  5. Find a balance between scalability and customization: \"A balance must be found between, on the one hand, adapting the hospital's processes to the way the software is designed, and on the other hand, adapting the software to the way the hospital's processes are carried out\".<\/li>
  6. Change management and training are ongoing: Initial training on how to use the software may not be sufficient. Change management must consider a phase of assisted operation.<\/li>
  7. Vision and leadership are essential: Leadership from the top management of the project is important during all implementation processes. \"Only with the understanding and support of the hospital management is it possible to get the team to collaborate for the necessary cultural change\".<\/li>
  8. User-centered design: The role of the user within the project, as well as including multidisciplinary teams are key elements for successful implementation.<\/li>
  9. Understand the ecosystem: Data interoperability must be managed from the beginning, as well as the integration of other systems; the EHR will not work if it is understood individually and in isolation.<\/li>
  10. Early wins are important to gain traction: Showing benefits from the beginning of implementation will be important to project stakeholders.<\/li><\/ol>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t

    Especialistas del BID enlistan recomendaciones y aprendizajes surgidos de la implementaci\u00f3n de un sistema de historias cl\u00ednicas electr\u00f3nicas. En febrero de est\u00e9n a\u00f1o, el Banco Interamericano de Desarrollo (BID), public\u00f3 la propuesta p\u00fablica VISI\u00d3N 2025 Reinvertir en las Am\u00e9ricas: Una D\u00e9cada de Oportunidades, que busca a trav\u00e9s de una econom\u00eda digital, facilitar el acceso a la adopci\u00f3n de tecnolog\u00edas digitales y fomentar la innovaci\u00f3n. En la publicaci\u00f3n Sistemas de Historias Cl\u00ednicas Electr\u00f3nicas Definiciones, evidencia y recomendaciones pr\u00e1cticas para Am\u00e9rica Latina y el Caribe, abordan temas como la eSalud, transformaci\u00f3n digital y experiencias sobre implementaci\u00f3n de EHR en sistemas de salud. Por otra parte, en una entrada de blog, el BID presenta como ejemplo la implementaci\u00f3n de un sistema de EHR, en el estado de Bah\u00eda, donde la Secretar\u00eda de Salud del Estado de Bah\u00eda (SESAB), utiliza un sistema de software libre llamado AGHUse. Mediante esta experiencia, Bertha Brice\u00f1o, Fernanda Camera, Jennifer Nelson y Luis Tejerina, especialistas del BID en Salud Digital y transformaci\u00f3n digital enumeraron diez recomendaciones o aprendizajes a tomar en cuenta sobre los sistemas de EHR: El software gratuito no existe: Los autores explican que los softwares abiertos no implican un gasto cero, ya que requiere de un equipo de profesionales que acompa\u00f1e su implementaci\u00f3n y mantenimiento. La gobernanza es clave: \u201cEn este formato de desarrollo comunitario de software, es importante tener un sistema de gobernanza que defina con claridad los roles y compromisos de cada socio, los procedimientos para solicitar ajustes y resolver problemas\u201d, explican los autores. Es importante crear un modelo sostenible y escalable: Se debe incluir un plan de sostenibilidad, establecer porcentajes de los desarrollos esperados. El software requiere adaptaciones, en especial para \u00e1reas especializadas: Los softwares utilizados deben ser seleccionados tomando en cuenta los procesos y necesidades particulares que utilizar\u00e1 cada departamento por ejemplo maternidad, oncolog\u00eda, servicios geri\u00e1tricos etc. Buscar balance entre escalabilidad y personalizaci\u00f3n: \u201cTiene que buscarse un balance entre, por un lado, adaptar los procesos que tiene el hospital a como est\u00e1 dise\u00f1ado el software, y por otro la adaptaci\u00f3n del software a la manera en que se realizan los procesos en el hospital\u201d. La gesti\u00f3n del cambio y la capacitaci\u00f3n son permanentes: La capacitaci\u00f3n inicial sobre el manejo del software puede no ser suficiente. La gesti\u00f3n del cambio debe considerar una etapa de operaci\u00f3n asistida. La visi\u00f3n y liderazgo son esenciales: El liderazgo desde la alta direcci\u00f3n del proyecto es importante durante todos los procesos de implementaci\u00f3n. “S\u00f3lo con la comprensi\u00f3n y el apoyo de la direcci\u00f3n del hospital es posible hacer que el equipo colabore para el cambio cultural necesario”. El dise\u00f1o centrado en el usuario: El rol del usuario dentro del proyecto, as\u00ed como incluir equipos multidisciplinarios son elementos clave para el \u00e9xito de la implementaci\u00f3n. Entender el ecosistema: La interoperabilidad de datos se debe gestionar desde el comienzo, as\u00ed como la integraci\u00f3n de otros sistemas, el EHR no funcionar\u00e1 si se entiende de manera individual y aislada. Las victorias tempranas son importantes para ganar tracci\u00f3n: Mostrar beneficios desde el comienzo de la implementaci\u00f3n ser\u00e1 importante para las partes interesadas en el proyecto.
