Article published in The Lancet, discusses the types of technologies used in response to the pandemic, as well as prior preparation and challenges in implementing new Digital Health strategies going forward.<\/h2>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t

Strategies for tracking, for infection detection, contact tracing and tracking, quarantine and isolation, and clinical management have evolved during the pandemic. And they have been used with different approaches in different countries and territories, however, always seeking similar objectives, to contain the virus and prevent further cases of COVID-19.<\/p>

As part of planning and monitoring, big data and AI have facilitated health emergency preparedness, in some cases succeeding in slowing or controlling the spread of the virus. In Wuhan, the city where the first cases were reported, health authorities used migration maps linked to cell phones, payment applications and social networks to collect data to track the movements of people who had visited the Wuhan market. Thanks to this strategy they developed machine learning models to make forecasts on virus transmission in the region and implement border controls and surveillance.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t

This also allowed territories such as Taiwan to initiate airline measures for travelers arriving from Wuhan. Taiwan responded effectively to the outbreak of COVID-19, which due to its proximity to China could cause a health crisis due to its high population density, being among the top 20.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t

With regard to infection detection, China also uses web-based tools to manage its resources and ensure that they reach the people who need them. In Taiwan, for example, thermal cameras in airports were used to quickly detect people with fever at the beginning of the outbreak.<\/p>

\u201cUnlike most other countries, Iceland has launched widespread testing of asymptomatic individuals. Using mobile technology, Iceland collects data on patient-reported symptoms and combines these data with other datasets such as clinical and genomic sequencing data to reveal information about the pathology and spread of the virus,\u201d noted the article published in The Lancet.<\/p>

Contact tracing and tracking is perhaps the tool that has been most quickly and easily applied digitally during the pandemic. In countries such as South Korea, cell phone positioning has been used to alert possible areas of infection: \u201cSouth Koreans receive emergency text alerts about new COVID-19 cases in their region, and people who could have been in contact with infected individuals are instructed to report to testing centres and self-isolate.\u201d<\/p>

However, as The Lancet article notes: \u201cContact tracing applications are not without pitfalls. Not all exposure requires quarantine, such as when the exposed individuals are wearing personal protective equipment or are separated by thin walls penetrable by mobile phone signals.\u201d<\/p>

The risks of applying Digital Health technologies for pandemic control do exist, and are also linked to economic inequalities: \u201cDigital health initiatives can amplify socioeconomic inequalities and contribute to health-care disparities,\" the authors explained in the article.<\/p>

The data on mass internet use worldwide is confusing, while in Europe 82% of the population used the internet during 2019, in Africa it was only 28%. \u201cTo effectively implement digital technology globally, interventions should be tailored to the target regions; broadband access requires federal and private sector investment in technology and infrastructure.\u201d<\/p>

In addition, some interventions could threaten on the privacy of their users, a point that governments must also take into account when implementing these solutions. Art\u00edculo publicado en The Lancet, analiza los tipos de tecnolog\u00edas utilizados en respuesta a la pandemia, as\u00ed como la preparaci\u00f3n previa y los desaf\u00edos en la implementaci\u00f3n de nuevas estrategias de Salud Digital a futuro. Las estrategias de seguimiento, detecci\u00f3n de infecciones, rastreo y seguimiento de contactos, cuarentena y asilamiento y gesti\u00f3n cl\u00ednicas han evolucionado durante la pandemia. Y han sido utilizadas con diferentes enfoques en diversos pa\u00edses y territorios, sin embargo, siempre buscando objetivos similares, contener el virus y evitar m\u00e1s casos de COVID-19. Dentro de la planificaci\u00f3n y seguimiento, los macrodatos y la IA han facilidad la preparaci\u00f3n ante la emergencia sanitaria, logrando en algunos casos frenar o controlar la propagaci\u00f3n del virus. En Wuhan, la ciudad donde se reportaron los primeros casos, las autoridades sanitarias utilizaron mapas de migraci\u00f3n vinculados a tel\u00e9fonos m\u00f3viles, aplicaciones de pago y redes sociales, de esta forma recopilaron datos para rastrear los movimientos de personas que hab\u00edan visitado el mercado de Wuhan. Gracias a esta estrategia desarrollaron modelos de aprendizaje autom\u00e1tico para realizar pron\u00f3sticos sobre la transmisi\u00f3n del virus en la regi\u00f3n e implementar controles y vigilancia fronteriza. Esto tambi\u00e9n permiti\u00f3 que territorios como Taiw\u00e1n, iniciaran las medidas en aerol\u00edneas para los viajeros que llegaban de Wuhan. Taiw\u00e1n respondi\u00f3 de forma eficaz ante el brote de COVID-19, que por su cercan\u00eda con China podr\u00eda provocar una crisis en salud por su alta densidad de poblaci\u00f3n al encontrarse entre los 20 primeros. Con respecto a la detecci\u00f3n de infecciones, China tambi\u00e9n utiliza herramientas web para administrar sus recursos y que lleguen a las personas que lo necesitan. En Taiw\u00e1n por ejemplo las c\u00e1maras t\u00e9rmicas en aeropuertos fueron utilizadas para detectar r\u00e1pidamente a personas con fiebre al inicio del brote. \u201cA diferencia de la mayor\u00eda de los dem\u00e1s pa\u00edses, Islandia ha lanzado pruebas generalizadas de personas asintom\u00e1ticas. Con la tecnolog\u00eda m\u00f3vil, Islandia recopila datos sobre los s\u00edntomas notificados por los pacientes y los combina con otros conjuntos de datos, como datos de secuenciaci\u00f3n cl\u00ednica y gen\u00f3mica, para revelar informaci\u00f3n sobre la patolog\u00eda y la propagaci\u00f3n del virus\u201d, menciona el art\u00edculo publicado en The Lancet. El rastreo y seguimiento de contactos es quiz\u00e1 la herramienta que ha sido aplicada digitalmente con mayor rapidez y facilidad durante la pandemia. En pa\u00edses como Corea del Sur se ha utilizado el posicionamiento de los tel\u00e9fonos m\u00f3viles para alertar sobre posibles zonas de contagio: “Los surcoreanos reciben alertas de texto de emergencia sobre nuevos casos de COVID-19 en su regi\u00f3n, y las personas que podr\u00edan haber estado en contacto con personas infectadas reciben instrucciones de presentarse a los centros de pruebas y aislarse”. Sin embargo, como apunta el art\u00edculo de The Lancet: \u201cLas aplicaciones de rastreo de contactos no est\u00e1n exentas de dificultades. No toda exposici\u00f3n requiere cuarentena, como cuando las personas expuestas usan equipo de protecci\u00f3n personal o est\u00e1n separadas por paredes delgadas penetrables por se\u00f1ales de tel\u00e9fonos m\u00f3viles\u201d. Los riesgos de la aplicaci\u00f3n de tecnolog\u00edas en Salud Digital para el control de la pandemia existen, y tambi\u00e9n est\u00e1n ligados a las desigualdades econ\u00f3micas: “Las iniciativas de salud digital pueden amplificar las desigualdades socioecon\u00f3micas y contribuir a las disparidades en la atenci\u00f3n de la salud”, explican los autores en el art\u00edculo. Los datos sobre el uso masivo de Internet en el mundo son enga\u00f1osos, mientras en Europa el 82% de la poblaci\u00f3n utiliz\u00f3 internet durante 2019, en \u00c1frica fue solamente el 28%. \u201cPara implementar eficazmente la tecnolog\u00eda digital a nivel mundial, las intervenciones deben adaptarse a las regiones objetivo; el acceso de banda ancha requiere inversi\u00f3n federal y del sector privado en tecnolog\u00eda e infraestructura\u201d. Adem\u00e1s, algunas intervenciones podr\u00edan atentar contra la privacidad de sus usuarios, un punto que tambi\u00e9n deben tener en cuenta los gobiernos al momento de aplicar estas soluciones. Sin embargo, hasta ahora las respuestas ante la emergencia sanitaria basadas en herramientas digitales han tenido resultados importantes.
