National Center on Health Information Systems (CENS), in Chile, published a study at the JMIR on the interoperability of reporting PCR test results, using FHIR standards.<\/h2>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t

Proyecto Experanza COVID-19 is a public interest initiative, promoted by the Faculty of Medicine of the Catholic University of Chile (UC), in collaboration with the UC CHRISTUS Health Network, CENS, and the Digital Health Unit of the South East Metropolitan Health Service (SSMSO). The project consists of the development of a system of PCR tests in a massive way, and thus achieve the isolation of people who are positive cases of COVID-19 and cut the chain of contagion, all through a system of interoperability between the parties involved.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t

CENS has been in charge of offering interoperable solutions for this project, first through three stages. Firstly, there is the web triage through the COVID-19 Early Detection and Symptom Monitoring Platform, an online self-assessment system, in which the user performs a test to detect COVID-19 symptoms.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t

CENS created a proof of concept (POC), which is based on HL7 FHIR as an interoperability standard, which facilitates the management of data on the sampling of the Family Health Centers Network and the results that come from the UC Clinical Laboratory, to the centralized organization that is represented by the SSMSO. In this way, through an authentication mechanism, the different institutions were able to exchange information.<\/p>

They also developed for the third stage the system for monitoring positive patients from the SSMSO and other hospitals of the Family Health Centers Network, for the exchange of data between them, in this way they were able to standardize the information on patient admissions, monitoring and discharge of patients in their quarantine, as well as contact tracking records. Approximately 830,000 people have benefited from this program.<\/p>

And recently, the Journal of Medical Internet Research (JMIR), published the interoperability work developed by CENS, entitled \u201cInteroperable HL7 FHIR platform to report PCR SARS-CoV-2 tests from laboratories to the Chilean government\u201d, in which they detail how the development and design of the interoperable platform was carried out, as well as the methodology used.<\/p>

\u201cThe methodology to design and develop the interoperable platform was composed of six well-structured stages: 1) Creation of a minimum data set for PCR SARS-CoV-2 tests, 2) Modeling process and end points where institutions exchange information, 3) Standards and interoperability design, 4) Software development, 5) Quality assurance and 6) Software implementation\u201d, explain in the publication.<\/p>

In the publication they also signed about the results obtained: \u201cThe performance of the platform for 1000 requests resulted in a response time of 240 milliseconds, the performance was 28.3 requests per second and the process management time was 131 milliseconds. The platform has an availability of 99.9%\u201d.<\/p>

As a conclusion, the authors emphasized the importance of the platform to simplify the reporting of PCR tests, reducing the time and probability of errors in counting positive cases. It continues to operate today and is open to the medical community and laboratories that require the system for reporting results in their PCR tests.<\/p>

Centro Nacional en Sistemas de Informaci\u00f3n en Salud (CENS), en Chile, public\u00f3 estudio en el JMIR sobre la interoperabilidad en el reporte resultados de pruebas PCR, mediante est\u00e1ndares FHIR. El Proyecto Esperanza COVID-19 es una iniciativa de inter\u00e9s p\u00fablico, impulsada por la Facultad de Medicina de la Universidad Cat\u00f3lica de Chile (UC), en colaboraci\u00f3n con la Red de Salud UC CHRISTUS, el CENS, y la Unidad de Salud Digital del Servicio de Salud Metropolitano Sur Oriente (SSMSO).  El proyecto consiste en el desarrollo de un sistema de pruebas PCR de forma masiva, y as\u00ed lograr el aislamiento de las personas que resulten casos positivos de COVID-19 y cortar la cadena de contagios, todo mediante un sistema de interoperabilidad entre las partes involucradas. El CENS se ha encargado de ofrecer soluciones interoperables para este proyecto, primero a trav\u00e9s de tres etapas. En primera instancia se encuentra el triage web a trav\u00e9s de la Plataforma Detecci\u00f3n Temprana y Vigilancia de s\u00edntomas de COVID-19, un sistema de autoevaluaci\u00f3n online, en el cual el usuario realiza un test para detectar s\u00edntomas de COVID-19. El CENS cre\u00f3 una prueba de concepto (POC), que se basa en HL7 FHIR como est\u00e1ndar de interoperabilidad, lo que facilita la gesti\u00f3n de datos sobre la toma de muestras de la Red de Centros de Salud Familiar y los resultados que provienen del Laboratorio Cl\u00ednico UC, hasta la organizaci\u00f3n centrar que es representada por el SSMSO. De esta forma a trav\u00e9s de un mecanismo de autenticaci\u00f3n las diferentes instituciones lograron intercambiar informaci\u00f3n. Tambi\u00e9n desarrollaron para la tercera etapa el sistema de seguimiento de los pacientes positivos del SSMSO y otros hospitales de la Red de Centros de Salud Familiar, para el intercambio de datos entre s\u00ed, de esta forma lograron estandarizar la informaci\u00f3n sobre el ingreso de los pacientes, el seguimiento y el egreso de los pacientes en su cuarentena, as\u00ed como los registros de rastreo de contactos. Aproximadamente, 830 mil personas han sido beneficiadas por este programa. Y recientemente Journal of Medical Internet Research (JMIR), public\u00f3 el trabajo de interoperabilidad desarrollado por el CENS, titulado \u201cPlataforma interoperable HL7 FHIR para reportar pruebas PCR SARS-CoV-2 de laboratorios al gobierno de Chile\u201d, en el cual detallan c\u00f3mo fue el desarrollo y dise\u00f1o de la plataforma interoperable, as\u00ed como la metodolog\u00eda empleada. \u201cLa metodolog\u00eda para dise\u00f1ar y desarrollar la plataforma interoperable estuvo compuesta por seis etapas bien estructuradas: 1) Creaci\u00f3n de un conjunto m\u00ednimo de datos para las pruebas PCR SARS-CoV-2, 2) Proceso de modelado y puntos finales donde las instituciones intercambian informaci\u00f3n, 3) Est\u00e1ndares e interoperabilidad dise\u00f1o, 4) desarrollo de software, 5) aseguramiento de la calidad y 6) implementaci\u00f3n de software\u201d, explican en la publicaci\u00f3n. En la publicaci\u00f3n tambi\u00e9n firmaron sobre los resultados obtenidos: “El rendimiento de la plataforma para 1000 solicitudes dio como resultado un tiempo de respuesta de 240 milisegundos, el rendimiento fue de 28,3 solicitudes por segundo y el tiempo de gesti\u00f3n del proceso fue de 131 milisegundos. La plataforma tiene una disponibilidad del 99,9%” Como conclusi\u00f3n los autores enfatizaron la importancia de la plataforma para simplificar la notificaci\u00f3n de las pruebas PCR, reduciendo el tiempo y la probabilidad de errores en el conteo de casos positivos. Actualmente contin\u00faa funcionando y est\u00e1 abierta a la comunidad m\u00e9dica y los laboratorios que requieran el sistema para informar sobre los resultados en sus pruebas PCR.
