The Federal Medical Council (CFM) seeks to make the emergency resolution approving the use of telemedicine during the health emergency due to COVID-19 now a permanent reality. <\/h2>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t

On 20 March last, the Ministry of Health authorized the use of telemedicine on a temporary basis during the COVID-19 pandemic, it was through the Official Journal of Union in Ordinance No. 467 that telemedicine's actions were established with \u201cexceptional and temporary nature, with the aim of regulating and operatizing measures to address the international public health emergency\u201d.<\/p>

Previously in Brazil, Law No. 13.979 had been enacted on 6 February, and this provided for the need to regulate and carry out measures to address the public health emergency, with the aim of reducing the movement of persons exposed to COVID-19, as explained in the article.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t

In addition, continuing on the same path, the CFM in CFM resolution No. 1.643 \/ 2002 recognized the ethical possibility of the use of Telemedicine on an exceptional basis and as long as measures to address COVID-19 continue:<\/p>

\u201cTelemedicine's actions for distance interaction may include preclinic care, care support, consultation, monitoring and diagnosis, through information and communication technologies, within the scope of the SUS (Single Health System), as well as in complementary health and bathing,\u201d says Article 2 of the resolution.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t

Article 5 mentions that physicians may, in the field of Telemedicine care, issue medical certificates or prescriptions in electronic means. However, Law No. 13.989 was subsequently amended, which removed some items such as the one that included the issuance of electronic prescriptions. This law was sanctioned by President Jair Bolsonaro on 15 April and decreed authorization in the use of telemedicine during the health emergency.<\/p>

Now, the Federal Council of Medicine is developing a project that proposes to promote an ethical, technical and safe standard for the practice of telemedicine and which will be discussed in the coming months.<\/p>

The First Vice-President of the CFM and Coordinator of the Special Commission, Donizetti Giamberardino Filho, mentioned that the first principle guiding the preparation of this new resolution focuses on the doctor-patient relationship: \"This is the main value of the medical profession, and it is through this relationship that the necessary trust is established. The gold standard is face-to-face care. Telemedicine has to come as technology, with the main aim of facilitating access.\"<\/p>

He also expressed the benefits that can be obtained through telemedicine: \"you can, for example, transfer knowledge between medical centers, or even prevent the movement of people who travel 400 kilometers just to have a prescription. These are things that can help improve the collective health system and the SUS.\"<\/p>

Another topic that has been studied since the Special Commission is medical ethics and the care of users' privacy and data. So, they suggest that prior to a remote consultation, a face-to-face is made to ensure a more effective diagnosis and prescription.<\/p>

Currently, the CFM Special Commission of 11 specialists evaluates more than 2,000 proposals made by doctors in the public health and private services system.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t

El Consejo Federal de Medicina (CFM) busca que la resoluci\u00f3n de emergencia que aprueba el uso de telemedicina durante la emergencia sanitaria por COVID-19, sea ahora una realidad permanente. El 20 de marzo pasado el Ministerio de Salud autoriz\u00f3 el uso de la telemedicina de manera temporal durante la pandemia de COVID-19, fue a trav\u00e9s del Diario Oficial de Uni\u00f3n en la Ordenanza N\u00b0 467 donde se estableci\u00f3 con \u201ccar\u00e1cter excepcional y temporal, las acciones de Telemedicina, con el objetivo de regular y operativizar las medidas para atender la emergencia de salud p\u00fablica internacional\u201d. Previamente en Brasil se hab\u00eda promulgado la Ley N\u00b0 13.979, el 6 de febrero, y esta contemplaba la necesidad de regular y realizar medidas para atender la emergencia de salud p\u00fablica, con el objetivo de reducir la circulaci\u00f3n de personas expuestas a COVID-19, como explica el art\u00edculo. Adem\u00e1s, continuando sobre el mismo camino, el CFM en la resoluci\u00f3n No. 1.643 \/ 2002 del CFM reconoc\u00edan la posibilidad \u00e9tica del uso de la Telemedicina con car\u00e1cter excepcional y mientras contin\u00faen las medidas para enfrentar el COVID-19: \u201cLas acciones de Telemedicina de interacci\u00f3n a distancia podr\u00e1n incluir atenci\u00f3n precl\u00ednica, apoyo asistencial, consulta, seguimiento y diagn\u00f3stico, a trav\u00e9s de tecnolog\u00edas de la informaci\u00f3n y la comunicaci\u00f3n, en el \u00e1mbito del SUS (Sistema \u00danico de Salud), as\u00ed como en salud complementaria y ba\u00f1o\u201d, indica el art\u00edculo 2 de la resoluci\u00f3n. En el art\u00edculo 5 menciona que los m\u00e9dicos podr\u00e1n, en el \u00e1mbito de la atenci\u00f3n de Telemedicina, emitir certificados o recetas m\u00e9dicas en medios electr\u00f3nicos. Sin embargo, posteriormente fue modificada la Ley N \u00b0 13.989, que removi\u00f3 algunos art\u00edculos como el que inclu\u00eda la expedici\u00f3n de recetas m\u00e9dicas electr\u00f3nicas. Esta ley fue sancionada por el presidente Jair Bolsonaro el 15 de abril y decretaba la autorizaci\u00f3n en el uso de telemedicina durante la emergencia sanitaria. Ahora, el Consejo Federal de Medicina se encuentra elaborando un proyecto que plantea promover un est\u00e1ndar \u00e9tico, t\u00e9cnico y seguro para la pr\u00e1ctica de la telemedicina y que ser\u00e1 debatido los pr\u00f3ximos meses. El Primer vicepresidente del CFM y coordinador de la Comisi\u00f3n Especial, Donizetti Giamberardino Filho, mencion\u00f3 que el primer principio que gu\u00eda la preparaci\u00f3n de esta nueva resoluci\u00f3n se enfoca en la relaci\u00f3n m\u00e9dico-paciente: \u201cEste es el principal valor de la profesi\u00f3n m\u00e9dica, y es a trav\u00e9s de esta relaci\u00f3n que se establece la confianza necesaria. El patr\u00f3n oro es la atenci\u00f3n presencial. La telemedicina tiene que venir como tecnolog\u00eda, con el objetivo principalmente de facilitar el acceso\u201d. Tambi\u00e9n expres\u00f3 los beneficios que se pueden obtener a trav\u00e9s de la telemedicina: \u201cpuede, por ejemplo, transferir conocimientos entre centros m\u00e9dicos, o incluso evitar el desplazamiento de personas que recorren 400 kil\u00f3metros solo para tener una receta. Son cosas que pueden ayudar a mejorar el sistema de salud colectiva y el SUS\u201d. Otro de los t\u00f3picos que se han estudiado desde la Comisi\u00f3n Especial es la \u00e9tica m\u00e9dica y el cuidado de la privacidad y datos de los usuarios. Por lo que sugieren que previo a una consulta a distancia, se realice una presencial para asegurar un diagn\u00f3stico y una receta m\u00e1s efectiva. Actualmente, la Comisi\u00f3n Especial del CFM formada por 11 especialistas, eval\u00faa m\u00e1s de 2 mil propuestas realizadas por m\u00e9dicos del sistema de salud p\u00fablico y de servicios privados.
